This request must be completed even if you are requesting a copy of your own records.

PICK UP - There is a per page fee of $0.60 for printed records that are requested for pick up, per NRS 629.061.

MAILED - The per page fee above applies to requests for mailed records, as well as the cost of postage, which will include the fee for certified return receipt to ensure confidentiality of records. 

FAXED - If you wish your records to be FAXED to another provider, there is no charge. 

FEES - All fees are payable prior to the printing via invoice that will be sent to requester.

TIME FRAME - Per Nevada state law, providers have 30 days to process requests, please plan accordingly.
