new appointments available starting

If you would like to begin your care with us, please     complete the following form. We will reach out to                you to initiate the intake paperwork.             

INTAKE PAPERWORK must be COMPLETED first,

     then REVIEWED before an appointment can be                                 scheduled.

Please note New Patients appointments consist of a package purchase to ensure safe and quality care.
 

       The package consists of payment for:

  • ONE 60-minute initial evaluation and

  • ONE 30-minute follow up two weeks later
